Small group health insurance open enrollment runs from November 15 through December 15 every year. During that window a carrier can't turn your business down for missing its employee participation minimum or its employer contribution minimum. Get a complete enrollment in by December 15, 2026, and your group plan starts January 1, 2027. Miss that date and the usual rules are back on the table. We hold every group to December 15, no exceptions.
If you've tried to set up a group plan before and got told "not enough of your people signed up," this is the month that answer goes away.
What are participation and contribution rules?
Most of the year, a carrier writing a small group plan wants two things from you.
- Participation: a set share of your eligible employees has to enroll. People who already have coverage elsewhere, like through a spouse or Medicare, usually count as valid waivers.
- Contribution: the business has to pay a set share of each employee's premium.
Those rules keep a carrier from insuring only the one or two employees who need care the most. They also stop a lot of small Oklahoma businesses cold. A six-person shop where half the crew is young and happy with no plan, or already on a spouse's plan, can fail participation through no fault of the owner.
Why are they waived from November 15 to December 15?
It's federal law. Under 45 CFR 147.104(b)(1)(i)(B), a carrier may limit a small group that can't meet its contribution or participation rules to one annual enrollment period, which runs November 15 through December 15. Read the other way around: in that window, the carrier has to let the group in.
The same rule sets the start date. An enrollment the carrier receives between the 1st and the 15th of a month takes effect no later than the 1st of the next month. So an enrollment received by December 15 starts on January 1.
Key dates for a January 1, 2027 start
| Date | What happens |
|---|---|
| Now through November 14 | Call us. We pull quotes, build the census and walk you through plan options. |
| November 15, 2026 | The window opens. Participation and contribution minimums are off. |
| December 1, 2026 | Our target for having every employee application in hand. |
| December 15, 2026 | Hard deadline. A complete enrollment has to be submitted. No exceptions. |
| January 1, 2027 | Coverage starts. |
Why are we strict about December 15? Because the carrier is. An application that is missing a signature or a census line on the 16th doesn't get a grace day. It waits a year for the next window, or it goes back under the normal participation and contribution rules. Don't plan on finishing paperwork the week of the deadline.
A worked example
Take a made-up Oklahoma City HVAC company with eight full-time employees.
- Two are covered on a spouse's plan and sign waivers.
- Three want the group plan.
- Three say no thanks.
Offering health insurance to your team?
Small group plans for 2 to 50 employees through Blue Cross Blue Shield of Oklahoma, with dental and vision.
Get a Group QuoteThat's three out of six employees who don't have other coverage. In March, a carrier looking for most of those six to enroll would likely decline the group. The owner also wants to pay only a small share of the premium to start, which may fall under the carrier's usual contribution floor.
Submit the same group between November 15 and December 15 and neither of those problems can be used to say no. The three employees who want coverage get it on January 1, and the owner sets the share the business pays at a level the budget can carry. You can raise it later as the business grows.
Who should use this window?
- Businesses with 2 to 50 employees that have never offered a plan because they couldn't hit participation.
- Owners who want to offer coverage but can't afford to pay half of everyone's premium yet.
- Groups that were turned down earlier this year and were told to wait.
- Contractors, restaurants, shops and offices trying to hire against bigger companies that already offer benefits.
Group dental and vision can be added at the same time, so your team gets the full package in one enrollment.
What we need from you before December 15
- A list of every employee: name, date of birth, home zip code and whether they want employee only, employee and spouse, employee and children, or family coverage.
- Who is waiving and why (spouse's plan, Medicare, a parent's plan).
- Proof the business has employees on payroll. In Oklahoma that is usually your most recent quarterly wage report.
- Signed employee applications or waiver forms for everyone eligible.
- Your choice of plan and how much the business will pay toward premiums.
Send us the first two items and we can start quoting right away. We handle the forms and the carrier submission, and our help costs you nothing extra.
How we handle it
We're an independent broker in Oklahoma City, and we quote small group medical plans through Blue Cross Blue Shield of Oklahoma, plus group dental and vision. We build the census, show you plans side by side with what the business and each employee would pay, and chase every missing signature before the deadline instead of after it. After you're enrolled, we stay on for renewals and claims questions. You can read more about how group plans work on our group health insurance page, or see affordable health insurance options for Oklahoma employees.

Frequently asked questions
When is small group health insurance open enrollment?
November 15 through December 15 every year. For 2026, a complete enrollment must be submitted by December 15, 2026, for coverage starting January 1, 2027.
Is there a minimum participation requirement for small group health insurance?
Most of the year, yes. Carriers set a minimum share of eligible employees who have to enroll. Between November 15 and December 15, a carrier can't decline a small group for missing that minimum.
Does the employer have to pay part of the premium?
Outside the window, carriers usually require the business to pay a set share. During the November 15 to December 15 window, the contribution minimum can't be used to turn the group down, so you can start with what your budget allows.
What if we miss the December 15 deadline?
You can still apply any month, but the carrier's normal participation and contribution rules apply. If you can't meet them, the next waiver window is November 15, 2027.
